A birth-date change is a different form and a different screen from a name change, and it carries rules a name change does not — including one warning the Government prints on the form itself that almost nobody reads.
The Government prints this on the back of its own form, and it is the single most important thing on this page. Instruction 8 states, in substance, that publishing a birth-date change notice in the Gujarat Government Gazette does not by itself give the person any right to change their date of birth in government or other records. It explains why: such a notice is an advertisement, and it is not an authorised record or proof of the changed date of birth. Where a notice has been given by an unauthorised person, the Government accepts no responsibility for it.
A name change publishes and, in practice, offices accept the gazette as the evidence of it. A date of birth is treated very differently. School records, service records, PF and passport authorities each have their own rules about whether and when a recorded date of birth can be corrected, and many will not act on a gazette notice alone. Instruction 4 on the form points the same way: it is the date on the birth certificate that is published as the correct one.
So if someone tells you a gazette entry will simply change your date of birth everywhere, they are overselling it. Ask them to show you where the Government says so. We would rather you know this before you spend ₹300 than after.
None of this makes the gazette notice pointless — it is a required or supporting step in plenty of cases, and it creates a public, dated record. It is just not the whole answer on its own, and we are not lawyers: if a specific office has told you what it needs, follow that.

The form and the affidavit must both be completed in Gujarati. It is printed as a note at the top of the form, and instruction 4 adds that birth-date changes are not published in English. An English affidavit is one of the easier ways to have a birth-date application sent back.
The form must be typed or written legibly, and only the prescribed format is accepted. It is addressed to the Manager, Government Press & Stationery, Reed Club Road, near the New Collector Office, Rajkot – 360 001. You can file it there by money order or cash, or fill it online after logging in at egazette.gujarat.gov.in and pay the fee online. The apply screen is Change of Birthdate.
Two of the instructions on the form set hard time limits. Miss either and the door closes:

Whether the application is accepted or rejected, the original affidavit and the other documents you submitted are not returned to you. Keep your own certified copies of everything before you file. This is instruction 12 on the form, and it surprises almost everyone.
Responsibility for any legal issue arising out of the gazette entry rests with the applicant — the form says so in instruction 7. That is another reason to have the dates and spellings checked by someone who files these regularly before the money is spent.
