What's happening
Your ABHA number (Ayushman Bharat Health Account, under the Ayushman Bharat Digital Mission) is usually created using your Aadhaar, so the name on your ABHA is drawn from your Aadhaar record. A mismatch appears when your Aadhaar name differs from the name on your hospital records, insurance policy, or the name you actually use β or when your name has changed and not every record was updated.
Why it happens
- Your Aadhaar and your insurance/hospital records show different spellings or surname order.
- Your name changed after marriage or by choice, but the health and identity records weren't all updated.
- The ABHA was created before a recent Aadhaar correction, so it carries the old name.
Is a Gujarat Gazette required?
It depends on the cause. If it's a minor spelling difference, aligning the underlying Aadhaar (and any insurance/hospital record) usually resolves it β no Gazette needed. If your name has genuinely changed and you need one consistent name across health, insurance and identity records, a Gazette is the clean way to establish it. We'll tell you honestly which applies.
How to fix it
- Decide the correct name you want across your health and identity records.
- Compare the records β Aadhaar, ABHA profile, insurance policy and any hospital record.
- Align the base record β correct Aadhaar (and insurance, if needed). For a genuine change, establish the new name via a Gazette and supporting affidavit.
- Update the ABHA profile once the source records agree.

Common mistakes to avoid
- Editing the ABHA while Aadhaar and insurance records still disagree.
