What Is a 'One and Same Person' Declaration?

Over the years, the same individual can end up recorded under two (or more) different names β€” a fuller name on a school certificate, a shorter one at the bank, an expanded initial on one card. A "One and Same Person" declaration is a formal statement that all these names refer to a single individual: you.
